Probis 5 is prescribed for:
- Hypertension (High Blood Pressure): Lowers BP and reduces cardiovascular risk.
- Angina Pectoris: Relieves heart-related chest pain by reducing cardiac workload.
- Heart Failure: Improves cardiac efficiency and reduces symptoms.
- Arrhythmias: Controls irregular heart rhythms.
- Post-Myocardial Infarction: Helps prevent future heart attacks and stroke.
- Migraine Prophylaxis: Reduces migraine frequency (off-label).
- Hyperthyroidism (Overactive Thyroid): Alleviates symptoms related to excessive thyroid activity.

Adults
Hypertension / Angina: Starting dose 5 mg once daily. If needed, increase to 10 mg, then up to 20 mg once daily.
Heart Failure:
- Week 1: 1.25 mg once daily
- Week 2: 2.5 mg once daily
- Week 3: 3.75 mg once daily
- Week 4: 5 mg once daily
- Week 8: 7.5 mg once daily
- Week 12: 10 mg once daily (max dose)
- Hepatic impairment (severe): Start at 2.5 mg/day; max 10 mg/day.
- Renal impairment (CrCl <40 ml/min): Start at 2.5 mg/day.
- Elderly: 2.5–20 mg/day (Hypertension), up to 10 mg/day (Heart failure).
Administration:
- May be taken with or without food.
- Take regularly at the same time each day.
- Swallow whole with water; do not crush or chew.

- Cardiogenic shock
- Overt cardiac failure
- Second or third degree AV block
- Marked sinus bradycardia
- Severe hypotension

- Class I antiarrhythmics (quinidine, disopyramide, propafenone): Increased AV conduction delay.
- Catecholamine-depleting drugs (reserpine, guanethidine): Risk of excessive sympathetic suppression.
- Clonidine: Avoid abrupt withdrawal (may worsen rebound hypertension).
- Digitalis glycosides: Increased risk of bradycardia.
- NSAIDs: May reduce the antihypertensive effect.
- Verapamil/Diltiazem: Additive myocardial depression and AV block.

- Use with caution in renal/hepatic impairment.
- May mask symptoms of hypoglycemia in diabetic patients.
- Avoid abrupt discontinuation – taper gradually to prevent rebound hypertension or angina.
- Not recommended in severe peripheral arterial disease or severe asthma/COPD.
- Use with caution during pregnancy (Category C) and lactation.
